Create My Practice

Creates a practice, managed here, with the caller its first member, and puts the named organisations in it: each one the caller owns (400 otherwise) and in no practice (409 otherwise), checked before anything is created. Their members stay as they are; a contractor organisation with no HMRC credentials of its own files with the practice's default set.
